Parents must complete, supply and/or bring the following items, which will be given to you by the director, before enrollment:
**Health appraisal signed by a physician (may be picked up at your child's pediatrician's office)
** STATE OF MICHIGAN Information card
** Medical form
** Child’s history
** Discipline Policy Form
** Federal food program application (if applicable)
** Registration
** 1st weeks tuition.
** Restrictions shall also be noted, i.e.
immunizations and UPDATED YEARLY
** Small child sized blanket
(sent home each Friday for
cleaning)
** One extra change of weather
appropriate clothes
for older children
two changes for younger children
